How to get back to Hans Freighter and Starkiller Base

I've been through the whole story mode for The Force Awakens and now i can't seem to figure out how to get back to Hans Freighter or the Starkiller Base to get some of the collectibles. They don't show up on the star map. So how do i get back? Any help is greatly appreciated.

The only way to go back to them is to replay the missions that take place in them.

Is there a way to tell where each mission is located?

They don't say where they're located but the missions with Han next to them are in Han's Freighter and the ones with Leia next to them are in Starkiller Base.
